Where can I find the necessary forms or paperwork to enter DUII diversion?Most courts use standard forms provided by the State. [Washington County uses their own version.] These forms can be found in the courtroom of your diversion appearance. Typically, your lawyer will provide these forms to you filled out for your review and signature. If you represent yourself, it will be your responsiblity to find, read, and fill out these forms. A link to the statewide forms can be found here. Washington county DUII diversion forms can be found here. Multnomah County uses the statewide forms as does Clatsop County and Wasco County.
